Family Preparedness
We help families get documents that allow a trusted person to make decisions for the children of the family when parents are absent due to injury, death, jail, or deportation.
The parents choose a designated person (usually an adult family member or trusted friend) who will be given the power to make medical, educational, financial, or other decisions on behalf of the children.
To start, please fill out this form so we can contact you. We will need your name and a phone number for either yourself or a trusted friend (cell phones preferred) so that we can contact you. If you prefer, we can also contact you by email.
For us to help you, you should be prepared to supply the following information:
Birth Parents identification: birth certificate, passport, U.S. Permanent Resident Card (Green Card), Employment Authorization Document (EAD/I-766), consular ID, or Social Security card. They should also have at least two documents (letters, bills) giving proof of their residential address.
Children's birth certificate.
Trusted friend or surrogate's identification.
