We help families get documents that allow a trusted person to make decisions for the children of the family when parents are absent due to injury, death, jail, or deportation.

The parents choose a designated person (usually an adult family member or trusted friend) who will be given the power to make medical, educational, financial, or other decisions on behalf of the children.

For us to help you, you should be prepared to supply the following information:

Birth Parents identification: birth certificate, passport, U.S. Permanent Resident Card (Green Card), Employment Authorization Document (EAD/I-766), consular ID, or Social Security card. They should also have at least two documents (letters, bills) giving proof of their residential address.

Children's birth certificate.
​
Trusted friend or surrogate's identification.
